Your hunger bar sits opposite your health bar and runs from 0 to 20, in half-point steps shown as drumstick icons. Eating food restores hunger points, along with a hidden saturation value alongside it, while your hunger drains as you do things that cost energy: sprinting, jumping, attacking, taking damage, breaking blocks, and even healing itself. In Survival, you can't eat most food while your hunger bar is already full; a few items like golden apples, honey bottles and chorus fruit are exceptions.
Saturation is the part you don't see directly — it's a buffer that gets used up before your visible hunger points start dropping. As long as you have saturation left, your hunger bar holds steady even while you're active; once saturation hits zero, further exhaustion starts eating into the hunger bar itself, and that's when the bar visibly shrinks.
Hunger's biggest effect is on healing. Whenever your hunger sits at 18 or higher, or you still have any saturation left even with a lower hunger number, your health regenerates naturally, healing 1 health point roughly every 4 seconds. That regeneration itself costs hunger or saturation to run, so a long fight followed by healing can visibly drain your food bar even if you never got hit.
Drop below 18 hunger with no saturation left, and that natural healing simply stops. Your health won't fall on its own at that point, but it also won't recover from damage until you eat and push hunger back up.
Two more thresholds matter as hunger keeps dropping. At 6 or below, you lose the ability to sprint entirely, and you won't get it back until hunger climbs back above 7 — a real problem if you're trying to outrun something. At 0, starvation kicks in: you start losing 1 health point roughly every 4 seconds, and that damage ignores armor, armor toughness, and even the Resistance effect completely.
Starvation isn't equally dangerous everywhere, though. On Easy difficulty it stops once your health drops to 10 and won't take you any lower. On Normal it stops at 1 health point, painful but survivable on its own. On Hard difficulty there's no floor at all; starvation alone can kill you outright if you never eat. Treat 0 hunger as an emergency rather than something to ride out, since any other damage on top of it is far more dangerous with your health already low.
No — on Normal, starvation damage stops once your health reaches 1. It can't finish you off by itself there, though any other source of damage while you're at 1 health certainly can.
